WHAT IS PERSONAL DATA?

After your initial consultation, we securely hold a completed Initial Consultation (IC) form and full treatment records via an online clinical management system called Cliniko. This form includes your name, address, date of birth, and contact details.

Cliniko ensures the security of your data through multiple layers of protection. Data is encrypted both in transit and at rest, minimising the risk of unauthorised access. Automatic backups are performed regularly, and data is stored in ultra-secure facilities with robust physical and virtual safeguards. We require your address, phone numbers, and email address to communicate with you effectively.

Marketing material may be shared if you opt-in via the IC form or subscribe directly. We will never send unsolicited marketing material. You can unsubscribe from marketing communications at any time by contacting us or using the unsubscribe link provided in our emails.

Health-Related Data:

Initial Consultation Form (IC): To provide effective, evidence-based, and safe treatment, we collect your full medical history. This ensures that our practitioners can offer the best clinical outcomes tailored to your needs. If you are uncomfortable sharing these details, please inform us, but note that without this information, treatment cannot be provided.

Treatment Records: These include notes from our practitioners about your assessments, diagnoses, and treatments. Records are maintained to comply with our insurance policy and legal obligations.

Retention Periods:Our insurance policy requires that we retain patient records for a minimum of seven years after the last treatment. For minors, records are kept until seven years after they reach the age of maturity (18). After these periods, records are deleted via the Cliniko system only if requested by the patient.

Sharing Data:Personal data may be shared under the following circumstances:

HOW AND WHERE DO WE HOLD YOUR DATA?

Access to your data is strictly limited to practitioners at Remedium Wellness. We take all appropriate steps to protect the confidentiality, integrity, availability, and authenticity of your data.
